Community Regional Medical Center's GPR Program participates in the ADEA Postdoctoral Application Support Service (PASS) and Postdoctoral Dental Matching Program (PDMP).

We accept applicants from fully accredited ADA dental schools within the US or Canada. All must have U.S. or Canadian citizenship (green card acceptable).

The Community Medical Centers Dental Residency Program must receive all materials by the November 15 deadline.

In order to allow enough time for processing,
submit items below through PASS by November 1:

• Dental school transcripts including the Spring 2020 semester

• National Board results

• Professional Evaluation Forms (two)

• Dean’s Institutional Evaluation

• Undergrad transcripts

• CV/Resume

Email the following materials directly to our
residency program by November 15:

• Completed GPR 1 Year Application

• Match number (enter on application form)

• 2x2 color identification photo (place on application) or attach as jpeg
