Financial Assistance

Community Medical Centers has a long history of giving back to our community. That's reflected in our generous financial assistance policies which help low-income patients who seek care at our hospitals. We're committed to providing compassionate, high-quality care to the community we serve.


If you don't have health insurance, our staff can help you locate government or private programs that may provide financial assistance. The first step is eligibility for a government-sponsored health program like Medi-Cal or Medicare. If you qualify, these programs can provide healthcare access and other social services for you and your family. For assistance, please call Patient Financial Services at (559) 459-3939 or (800) 773-2223 ext. 53939.


Hospital Bill Complaint Program

The Hospital Bill Complaint Program is a state program, which reviews hospital decisions about whether you qualify for help paying your hospital bill. 


If you believe you were wrongly denied financial assistance, you may file a complaint with the Hospital Bill Complaint Program. Please visit the California Department of Health Care Access and Information for more information and to file a complaint.

3 Ways to Submit the Financial Assistance Application

If you're not eligible for the above, or are insured with high medical costs, you may qualify for Community Medical Centers' Financial Assistance program. 


We offer three ways to submit the assistance application. For questions or assistance with the process, call Patient Financial Services at (559) 459-3939 or (800) 773-2223 ext. 53939, or email us at CMCRevenueSystemsSupport@communitymedical.org.

How to apply:


  1. Log in to your MyChart account.
  2. Click 'Menu' in the top left corner of the screen (may appear as three horizontal lines only on mobile).
  3. Scroll down to 'Billing' and select 'Financial Assistance.'
  4. Follow the directions on your screen and upload the specified documents to complete the application.


How to apply:


  1. Print the Financial Assistance Application or pick one up at the Admitting Department at Community Regional Medical Center, Clovis Community Medical Center, or Fresno Heart & Surgical Hospital.
  2. Complete the application.
  3. Mail your completed application and documents to Patient Financial Services, Community Medical Centers, PO Box 1232, Fresno, CA 93715-9889.

How to apply:


  1. Print the Financial Assistance Application or pick one up at the Admitting Department at Community Regional Medical Center, Clovis Community Medical Center, or Fresno Heart & Surgical Hospital.
  2. Complete the application.
  3. Fax your completed application and documents to (559) 230-8505.

Covered and non-covered provider list

Providers whose services are covered under our Financial Assistance Policy include:

  • Nurses who do not have advance practice licenses

  • Certified nursing assistants, medical assistants and other non-licensed assistants

  • Physical therapists, occupational therapists, speech language therapists and therapy assistants

  • Pharmacists

  • Technologists or technicians

  • Laboratory scientists

  • Registered dietitians

  • Diabetes educators

  • Audiologists and hearing aid dispensers

  • Any other non-licensed staff

 

Providers whose services are covered under our Financial Assistance Policy only when employed by Community Medical Centers include:


  • Nurse practitioners

  • Physician assistants

  • Certified nurse midwives

  • Licensed clinical social workers and licensed marriage family therapists

  • Clinical nurse specialists

  • Perfusionists

 

The list of Non-Covered Providers within the facility who provide Emergency Medical Care or other Medically Necessary Care at Community Medical Centers facilities is maintained in a supplemental document called “Medical Staff Roster with Primary Office.” This list is updated quarterly, is published here on our website and can be provided, free of charge, upon request.
