PGY-1 Pharmacy Residency Program Director
Dr. Sabrina Mahal is the PGY-1 Pharmacy Residency Program Director at Community Medical Centers. She serves as a preceptor to PGY1 pharmacy residents and pharmacy students.
Dr. Mahal earned her Bachelor of Applied Arts and Sciences degree from University of the Pacific, and her Doctor of Pharmacy from University of the Pacific, Thomas J. Long School of Pharmacy.
She has also completed a PGY1 pharmacy residency at Community Medical Centers and subsequently obtained Board Certified Pharmacotherapy Specialist certification.
She currently has memberships with the American Society of Health-System Pharmacists (ASHP) and California Society of Health-System Pharmacists (CSHP).

Dr. Lu is a critical care clinical pharmacy specialist in the Medical Intensive Care Unit. She serves as a preceptor for PGY1 pharmacy residents, as well as pharmacy students from the University of California, San Francisco, Thomas J. Long School of Pharmacy and Health Sciences at the University of the Pacific, and California Health Sciences University.
Dr. Lu received her Bachelor of Science in Chemistry from the University of Hawaii, Manoa, in 2006, and Doctor of Pharmacy from the University of California, San Francisco, in 2010. Dr. Lu completed her PGY1 pharmacy residency at Community Regional Medical Center in 2011.
Dr. Lu was honored with the Longs Foundation Award for Excellence in Teaching (“Preceptor of the Year”) for the UCSF Fresno site in 2015 and 2018, and was honored with the UCSF Apple for Excellence in Teaching award in 2017 and 2019. The Longs Award is the highest teaching award given by the UCSF School of Pharmacy.

Dr. Nadine Musallam is a critical care clinical pharmacy specialist and rounds in the Medical Intensive Care Unit. She serves as a preceptor for PGY1 pharmacy residents. Dr. Musallam received her Bachelor of Science in Human Biology from the University of California, San Diego, and Doctor of Pharmacy from the University of Southern California.
She completed her PGY1 in pharmacy practice and PGY2 in critical care at New York University Langone Medical Center. After residency, she served as a Medical ICU specialist at Mount Sinai in New York City until joining Community Regional.

Dr. Vu is a clinical pharmacy specialist in Trauma-Surgery Critical Care. She serves as an critical care rotation preceptor for PGY1 pharmacy residents as well as an adjunct clinical faculty preceptor for UCSF, UOP, and CHSU pharmacy students on their APPE acute care and elective rotations.

Dr. Mallory Cruz is a clinical pharmacy specialist in Emergency Medicine. She serves as a preceptor to PGY1 pharmacy residents, University of California, San Francisco pharmacy students, Thomas J. Dr. Cruz received her Bachelor of Arts in chemistry from California State University, Fresno in 2007 and Doctor of Pharmacy from University of California, San Diego, Skaggs School of Pharmacy in 2011. She completed a PGY1 pharmacy residency at Community Medical Centers in 2012.

Dr. Ibarra is a clinical pharmacy specialist in Emergency Medicine at Community Regional Medical Center. Long School of Pharmacy and Health Sciences at the University of the Pacific, and California Health Sciences University. Additionally, Dr. Ibarra is an Adjunct Assistant Professor at the CHSU College of Medicine.
Dr. Ibarra completed a PGY2 Critical Care Pharmacy Residency at The Johns Hopkins Hospital, PGY1 Pharmacy Residency at Stanford Health Care, and graduated from UCSF. He obtained a BA in Psychology and Minor in Education and Applied Psychology from the University of California, Santa Barbara.
Dr. Ibarra recently received the following awards: 2019 CRMC Exceptional Mentorship, 2020 UCSF Apple for Excellence in Teaching, 2021 CRMC Preceptor of the Year, 2021 UCSF Apple for Excellence in Teaching, 2022 Community Medical Centers Most Versatile Preceptor, and 2022 UCSF Fresno APPE Preceptor of the Year.
Dr. Kelly Oldziej is a clinical pharmacy specialist in emergency medicine at Community Regional Medical Center, serving as a preceptor to PGY-1 pharmacy residents and pharmacy students.
Dr. Oldziej received her Bachelor of Science in biology from University of Florida in 2016 and Doctorate of Pharmacy from University of South Florida Taneja College of Pharmacy in 2021. Dr. Ford is a clinical pharmacist at Clovis Community Medical Center who precepts students and residents in Emergency Medicine. Dr. Ford is native to the Fresno area; he attended Fresno State before earning his Pharm.D. at Pacific University in Oregon.
He returned to the valley to complete his PGY1 residency at Valley Children’s Hospital before coming on staff at Clovis Community.
Dr. Ford enjoys precepting and research. He has been voted the Clovis Community Medical Center Pharmacy Residency Preceptor of the Year three times.

Dr. Stanley Snowden is a clinical pharmacist in the inpatient pharmacy. Dr. Snowden received his Doctor of Pharmacy from the University of New Mexico, College of Pharmacy. He completed his PGY1 in pharmacy practice and Fellowship in cardiology at the University of New Mexico Medical Center.
After completion of his fellowship, he served as a faculty member at California Health Sciences University (CHSU), College of Pharmacy and an ambulatory care pharmacist at a Federally Qualified Healthcare Clinic in Madera, CA, until joining Community Regional.
Dr. Pang Ko is a clinical pharmacist in the inpatient pharmacy. He received his PharmD from Ohio State University, College of Pharmacy. He has been practicing at Clovis Community Medical Center since 2018.
Dr. Veerpal Sandhu is a clinical pharmacist at Community Regional Medical Center. He serves as a preceptor for PGY1 pharmacy residents in the hospital practice rotation and longitudinal projects.

Dr. Patel completed his pharmacy training at Massachusetts College of Pharmacy and Health Sciences University in Boston, MA, in 2021, and completed a PGY1 residency with Community Medical Centers in 2022.

Dr. Tyler Fallert is the Medication Safety Coordinator for Community Regional Medical Center. He serves as a preceptor for PGY1 pharmacy residents and pharmacy students. Dr. Fallert received his Bachelor of Science in Biochemistry from California State University San Marcos in 2015 and Doctor of Pharmacy from University of Pacific Thomas J. Long School of Pharmacy and Health Sciences in 2019.

He then pursued both Doctorate of Pharmacy and Masters of Business Administration degrees at the University of the Pacific. Dr. Weber loved Stockton so much, he decided to pursue a PGY-1 residency at St. Joseph’s Medical Center.
Prior to joining Clovis Community, he served as a pharmacist at Kaweah Health for more than 12 years in various areas, including internal medicine, emergency medicine, critical care and oncology. He is a member of American Society of Health-System Pharmacists.

Fouad practices in the ambulatory care setting as a clinical oncology pharmacist under Community Medical Oncology Specialists at Community Cancer Institute. Fouad has been in this role for 5 years providing treatment consultations to patients, clinical trial services, as well as clinic & provider support.

He currently precepts APPE students from California Health Sciences University (CHSU) in both Ambulatory and Elective rotations, as well as serving as a preceptor to Community Medical Centers PGY1 pharmacy residents. Dr. Husted received his Doctor of Pharmacy from University of the Pacific School of Pharmacy and Health Sciences in 2005. Dr. Husted completed his Pediatric Pharmacy Practice Residency at Valley Children’s Hospital in 2006. He also earned his Master’s Degree in Business Administration from CSU Fresno in 2017. Dr. Husted was awarded the UCSF Apple for Excellence in Teaching Award for the UCSF Fresno Pharmacy site in 2011 and the Outstanding Non-Physician Teaching Award from the UCSF Fresno Medical Residency Program in 2020.
He currently has memberships with the California Society of Health-System Pharmacists, Pediatric Pharmacy Advocacy Group, and American Society for Parenteral and Enteral Nutrition where he holds a number of leadership and committee member roles.

Dr. Ip is a clinical pharmacy specialist in neonatal intensive and pediatric intensive care units at Community Regional Medical Center. She received her Bachelor of Science in neurobiology and physiology from University of Maryland College Park and Doctorate of Pharmacy from Howard University in Washington, D.C.
Dr. Ip completed her PGY1 pharmacy residency at Howard University Hospital in 2010 and PGY2 Emergency Medicine pharmacy residency at Gaston Memorial Hospital in 2011.

Dr. Megan Ikeda is a clinical pharmacist at Clovis Community Medical Center. She serves as a preceptor to Community Medical Center Centers PGY1 pharmacy residents for the Women and Children’s rotation.
Dr. Ikeda received a Bachelor of Science in Neurobiology from the University of California, Irvine, before earning a Doctor of Pharmacy from The Ohio State University. She is a native to the Fresno area and came back to the Valley to complete her PGY-1 pharmacy residency at Valley Children’s Hospital.

Hospital Administration
Dr. Royston is the System Director of Pharmacy Services and precepts PGY1 residents as well as pharmacy students. Dr. Royston received her Bachelor of Science in biochemistry and cell biology and her Master of Science in biomedical science at University of California, San Diego.
She received her Doctor of Pharmacy at University of California, San Francisco. She completed a general pharmacy practice residency at University of Arizona and a critical care pharmacy practice residency at Mayo Clinic.
Dr. Royston worked as a clinical pharmacy specialist and clinical pharmacy manager until 2012. Since then, she has had the opportunity to work in several departments outside of pharmacy – Quality Management, High Reliability Culture and Supply Chain Management. During that time, she received her master of healthcare administration from National University and became a Certified Professional of Healthcare Quality.

Long School of Pharmacy and Health Sciences at the University of Pacific pharmacy students.
Dr. Lopez received his Bachelor of Arts in chemistry at California State University, Fresno, and his Doctor of Pharmacy at University of California, San Francisco. Dr. Dunn started his career as an inpatient pharmacist at Kaiser Permanente Fresno and Clovis Community Medical Center in 2012.
Dr. Kwan is an Inpatient Pharmacy Supervisor for Community Regional Medical Center. Dr. Kwan received his Bachelor of Science in Chemical/Biochemical Engineering from the University of California, Davis in 2002 and his Doctor of Pharmacy from University of the Pacific Thomas J. Long School of Pharmacy in 2009. Dr. Kwan also received a Masters in Business Administration from the California State University of Fresno, Craig School of Business in 2022.
