Community Regional Medical Center’s Transfer Center is staffed by highly-qualified registered nurses is available 24 hours a day, seven days a week. It facilitates and streamlines:

• Transfers from other hospitals
• Direct admission requests from physician offices
• Direct admission requests from clinics

Step 1: Fill out the appropriate form.

Step 2: Call the Transfer Center at (559) 459-5555.

Please be ready to provide the following information:

• Your name
• Your contact number
• The name of the physician requesting the transfer
• The name of the facility requesting the transfer
• Diagnosis of patient
• Reason for transfer
• Name and age of patient
• Level of care needed (intensive care unit, floor, etc.)
• Service line needed (trauma, cardiology, neurosurgery, etc.)
