How to make referrals

You will need to provide a patient referral if you have a patient that is interested in Diabetes Management Services.

Referrals can be made in two ways:

• Through Epic EMR
• By completing the appropriate referral form for either pregnant patients or non-pregnant patients and faxing it to (559) 459-1034
