Recognized by the American Diabetes Association, the Community Diabetes Education team will work with you to manage your diabetes by creating a personalized care plan, based on your unique health history and lifestyle. This care plan will provide tailored education and treatment to aid in preventing diabetes-related complications.

This comprehensive diabetes education and treatment program works with women before they get pregnant, during their pregnancies, and following the delivery of the baby to help ensure a happy and healthy child birth.
Participants receive a diabetes assessment, meter training and 10 hours of diabetes education. The program also provides individual medical nutrition therapy for patients with chronic renal insufficiency as well as diabetes mellitus.
The Community Diabetes Education Center offers Diabetes on Demand services to existing patients. On Demand service provides education and support when you need it most and at your convenience.
