Serving the underserved has been part of our mission for more than 125 years. Each year, we invest millions of dollars throughout our region to improve the community’s overall health and wellbeing based on community input in a community health needs assessment (CHNA), which is conducted every three years in collaboration with local hospitals and public health departments.

We collect information from community members and data from state and national resources to determine which health needs hold the most significance in our four-county primary service area.

We work to address these needs through hospital-funded initiatives and strategic partnerships with community-based organizations to extend our impact beyond the services we provide to our patients in our facilities.

2023 Community Benefit Report

The Community Benefit Report is a state-mandated report due annually to the California Department of Health Care Access and Information (HCAI), formerly the Office of Statewide Health Planning and Development (OSHPD), which also demonstrates Community’s commitment to investing in our region's growing health needs.

Community has historically spent more on uncompensated community benefit than all other Fresno-area hospitals combined.

Health Needs Assessment

2022 Community Health Needs Assessment

The 2022 Community Health Needs Assessment (CHNA) is a collaborative effort between Hospital Council of Northern and Central California and eight regional hospitals and public health department partners. Nearly 5,000 community members, leaders and key stakeholders representing low-income and vulnerable populations provided primary data through focus groups, one-on-one interviews and an online survey. Secondary data reported by federal and state sources was also referenced.

Included in the CHNA is our Evaluation of Impact report. At the end of the three-year period, we report on the impacts we made to address the identified health needs in our region through the detailed initiatives in our previous Implementation Strategy. This evaluation of impact outlines our efforts to address the health needs that were identified in the 2019 CHNA.

Implementation Strategy

2023–2025 Fiscal Year Implementation Strategy

The Fiscal Year 2023-2025 Implementation Strategy uses our 2022 Community Health Needs Assessment (CHNA) to identify and prioritize the health needs that we will primarily focus our Community Benefit activities and resources on for fiscal years 2023-2025.

The identification and prioritization of health needs was done in partnership with local community organizations through strategic planning and prioritization meetings.

The Implementation Strategy is the third part of the federally-mandated report required of all tax-exempt hospitals every three years.
