Community Subacute and Transitional Care

Subacute Care

Our subacute and skilled care center is an alternative to hospitalization for patients who are in a coma or require 24-hour medical supervision for long periods of time. Simply awakening – something most of us take for granted – can be a very meaningful experience for patients and their loved ones, and our staff devotes every effort to helping our residents make conscious responses. We also help our patients begin the rehabilitation process.

Community Subacute & Transitional Care Center

Helping patients and families since August 1992, our 106-bed long term care facility specializes in care for chronic, subacute conditions that includes ventilator, respiratory, wound, IV, hospice, rehabilitation services and general nursing care.

Our staff values individualized treatment for the physical, emotional and spiritual needs of our residents and is committed to carrying out these objectives with compassion and respect.

Home remedies: Fact or fiction?

MedWatch the TV show is calling for viewers to dust off their cameras and send in a video, e-mail or letter with their home remedy and it may be featured in an upcoming segment.

If you have a home remedy you think could help others, send it to us. We’ll put it to the MedWatch test and a Community Medical Centers doctor will see if it’s “Fact or Fiction.”
