Our Transfer Center facilitates and streamlines both transfers from other hospitals and direct admission requests from physician offices and clinics. It is staffed by highly-qualified registered nurses is available 24 hours a day, seven days a week.

To admit a patient from a physician’s office/clinic to Community Regional

Step 2: Call the Transfer Center at (559) 459-5555.
Please be ready to provide the following information:
• Your name
• Your contact number
• The name of the physician requesting the transfer
• The name of the facility requesting the transfer
• Diagnosis of patient
• Reason for transfer
• Name and age of patient
• Level of care needed (intensive care unit, floor, etc.)
• Service line needed (trauma, cardiology, neurosurgery, etc.)
