Full- or part-time (working more than 40 hours per pay period) employees are eligible for medical/dental coverage on the first day of the month following 30 days of employment and can choose from the following options.
Employees can choose from three medical plans, including an HMO and two PPO options. All three plans include coverage in Community's prescription drug plan.
Community offers two combined dental and vision plans: a traditional plan/Preferred Provider Organization (PPO) and Dental Health Maintenance Organization (DHMO) dental and vision plan.
Optional Term Life Insurance
Employees have the option to purchase additional term life insurance at a reasonable cost for themselves and their family.
Community's basic long-term disability plan provides 60% of an employee's base pay.
