We offer a full continuum of care, including home health and palliative care. Our goal is to support both you and your family – in the comfort of your own home – by offering an alternative to frequent and prolonged hospital stays.
In the privacy of your home, we offer skilled nurses, therapists (physical, speech and occupational), registered dieticians, nursing aides and medical social services. All services are provided under the direction of your physician, as an extension of the physician’s care. The duration and frequency of our services are determined by your individual needs, and we’re available 24 hours a day, seven days a week.
Our palliative care meets the needs of terminally and chronically ill patients who live at home and need in-home nursing, therapy and pain-management services. We aim to be sensitive to personal, cultural and religious beliefs and to help you achieve the best possible quality of life by relieving pain, controlling symptoms and restoring functional abilities.
Palliative care is not the same as hospice care. Hospice is rendered at the end of life, whereas palliative care might encompass the entire continuum of life. Palliative care is for patients who do not qualify or do not desire hospice care and ultimately supports patients and families as they transition into hospice.
Our registered nurses, nurse specialists, social workers, dieticians and therapists (physical, speech and occupational) educate, support and provide quality infusion care. The therapies we provide in-home would otherwise require hospitalization. Our services can also be provided in long-term or skilled nursing facilities.
