The Cedar Avenue Campus of Community Medical Centers Dental Residency Program must receive the following materials, directly or through PASS, by November 15, 2008:
Send through PASS:   
• Dental school transcripts from a fully accredited ADA program (sent from the Registrar's Office) including the Spring 2008 semester    
• National board scores
• Curriculum vitae
• Dean's professional evaluation with comments of recommendation
• Faculty professional evaluation with comments of recommendation (at least two)

Mail the following materials directly to our residency program:      
• Completed resident application form
• Match number (enter on application form)
• Undergraduate transcripts
• 2x2 color identification photo
