As a hospital-based program, Community’s home care offers a full continuum of care from hospital to home, including home health and palliative care. Home care includes support for the patient and the patient’s family in the comfort of their own home, and offers an alternative to frequent and prolonged hospital stays.

Home Health
Home Health offers skilled nursing, physical, speech and occupational therapists, registered dieticians, nursing aides and medical social services. All services are provided in the patient’s home under the direction of a patient’s physician and are an extension of the physician’s care. The duration and the frequency of our services depend on the needs of the individual. Services are available 24 hours a day, seven days a week.

Palliative Care
Palliative Care meets the needs of Community Home Care Services’ terminally and chronically ill patients who live at home and are in need of in-home nursing and therapy services. Palliative care offers in-home pain management control for home bound, chronically ill patients who may or may not have a deteriorating medical condition requiring symptom management.

The goal of palliative care is to achieve the best possible quality of life through relief of suffering, control of symptoms and restoration of functional capacity while remaining sensitive to personal, cultural and religious beliefs and practices. Palliative care is distinguished from hospice in that hospice is rendered at the end of life, whereas palliative care may encompass an entire continuum of life. This care is for patients who do not qualify or do not desire hospice care and ultimately supports patients and their families as they transition into hospice.

Home Infusion
Community home infusion provides services in conjunction with home health and serves patients’ health care needs safely in the comfort of non-hospitalized settings.

We educate, support and provide quality infusion care for our regional communities. The support consists of registered nurses, nurse specialists, social workers, dieticians and physical, speech and occupational therapists. We work in cooperation with the patients, payers, physicians and other health care team providers.

Community Home Infusion provides many different therapies for the treatment of illnesses that would otherwise require hospitalization. Our trained pharmacists work closely with the patient’s physician to ensure timely and precise care.

The skilled health care professionals provide care primarily in the comfort and familiarity of the patient’s home where they can be supported by friends and family. Our services also can be provided in an alternative site setting (i.e. long-term care or skilled nursing facilities).

Services are provided 24 hours a day, seven days a week.
