Medical and Dental Benefits

FlexChoice
Full-time or part-time (more than 40 hours worked per pay period) employees are eligible to choose from the benefit options listed below. Employees are eligible to receive medical/dental insurance coverage on the first day of the month following 30 days of employment.

Medical Plans
Employees can choose from three medical plans, including an HMO and two PPO options. All three plans include automatic vision care coverage, and coverage in Community's prescription drug plan.

Dental Plans
Community offers two dental plans: a traditional plan/Preferred Provider Organization (PPO) and Dental Health Maintenance Organization (DHMO) dental plan.

Health Reimbursement Account
Employees can receive reimbursement for eligible medical, dental and vision care expenses (for themselves or a family member) not paid by any additional health coverage they may have. This is done on a pre-tax basis.

Dependent Care Reimbursement Account
Employees can set up a reimbursement account for childcare expenses they incur. This is done on a pre-tax basis.

Long-Term Disability
Community's basic long-term disability plan provides 50% of an employee's base pay. A "buy-up" plan  provides 66.6%, or 2/3, of an employee's base pay.

Optional Term Life Insurance
Employees have the option to purchase additional term life insurance at a reasonable cost for themselves and their family.
