Subacute Care

Waking up every morning is something most of us take for granted, but for residents of Community Subacute Care Center-DeWitt, it can be a lifelong struggle.

Community’s DeWitt Subacute Center is a 33-bed alternative to hospitalization for patients who require 24-hour medical supervision for extended periods. DeWitt, located on the third floor of the old University Medical Center, specializes in chronic respiratory, neuro, cardiovascular, diabetes and other complex illnesses that require a tracheotomy, ventilator support, gastrostomy, IV therapy, wound healing, skin maintenance and pain management.

We consider it a blessing every time a resident “wakes” to make a conscious response and we devote our every effort to helping that happen.

The dedication and excellence of our staff has been recognized by several nursing and health associations across the state including the California Association of Health Facilities (CAHF), Hospital Services of Continuing Care (HSCC), and the Central Valley Regional Nursing Home Quality Initiative, Lumetra.

Our accomplishments have brought many awards in recent years, but we get our greatest satisfaction from maintaining a family-like atmosphere among our team while operating at such high standards. 

The Dewitt staff values teamwork, compassion and commitment to quality care. We provide individualized interdisciplinary care plans with strong family involvement. We have created a neighborhood environment where residents, staff, families and visitors feel the comforts of home.

The center is an active member of the Hospital Services of Continuing Care, the California Association of Health Facilities and a participant of Lumetra Nursing Home Collaborative Projects.
