Community Medical Centers recognizes that being in a hospital can be a very difficult and challenging experience. Social workers are available (free of charge) to assist you and your family, while coping with psychological and social problems that you may encounter when faced with an illness, hospitalization and medical treatment.
The Social Work Services Department provides services to all age groups in the inpatient, outpatient clinics and emergency department. We also have social workers available in: Leon S. Peters Rehabilitation, Home Health, Special Services and skilled nursing and subacute and dialysis centers.
Our social workers serve as patient advocates, counselors and problem solvers. When appropriate, social workers also will work with you and your family to help arrange for appropriate care after leaving the hospital. This often includes: rehabilitation hospitals, skilled nursing facilities, board and care, and hospice care.
Some of the unique programs that we provide are crisis intervention for trauma incidents and a bereavement program for families. Free bereavement literature is available to help families cope with grief and provides tools to help surviving children as they experience their own grief. All services are free of charge and are available through Community’s Social Work Services Department.
