The Community Diabetes Care Center offers diabetes management and community services through motivational programs, screenings, educational literature, symposia and rural outreach. Our diabetes care team provides personalized assessment, education, counseling and treatment to help patients self-manage their blood glucose levels.
The following programs are offered through the Community Diabetes Care Center:
Diabetes Management Program
Offers individual assessment and education on lifestyle changes, self-management classes and follow-up visits for those with types 1 and 2 diabetes.
Diabetes in Pregnancy Program
Offers valuable training for women with diabetes who desire to become pregnant and for expectant mothers with diabetes.
