Community's Asthma Education Program

Our asthma program plays an important role in helping patients achieve long-term control of their asthma symptoms and has been recognized by the Centers for Disease Control and Prevention as a national model.

The program aims to improve the quality of life for patients and their families by increasing their knowledge about asthma to reduce symptoms and emergency department visits.

Our program teaches patients:

  • The impact the environment has on asthma.
  • How to use the provided information, medication and tools to control their asthma.
  • To create active partnerships with their primary care physicians.

Upon a doctor referral to the program, the patient and family will receive basic education, typically in the form of a group setting.

At the conclusion of the class, each patient is encouraged to follow up with his/her physician.

Individual appointments, follow-up and case management will be provided by The Community Asthma Program to patients who have specific needs and/or following the request of their physician. During these visits, concerns and medications are addressed as well as the creation of a personalized asthma action plan. This plan will provide the appropriate steps to help keep the patient’s asthma under control.
