DEADLINE FOR APPLICATION:  JANUARY 15 (All application materials must be received in my office before January 15)
Applicants must have the following:

  • Pharm. D. degree from an ACPE accredited school of pharmacy
  • Apply for ASHP Residency Matching Program (our Match # 123313)
  • Offical Copy of transcripts
  • Completed online application
  • Three professional letters of recommendation (see form link on the left)
  • Copy of curriculum vitae or resume (electronic copy via email)
  • Letter of intent - describing reasons for seeking residency (electronic copy via email)
  • Licensed or eligible for licensure as a pharmacist in the state of California
  • If selected, the dates for on-site interviews are February 5 and 12, 2010
