Applicants must have the following:
- Pharm. D. degree from an ACPE accredited school of pharmacy
- Apply for ASHP Residency Matching Program (our Match # 123313)
- Offical Copy of transcripts
- Completed online application
- Three professional letters of recommendation (see form link on the left)
- Copy of curriculum vitae or resume (electronic copy via email)
- Letter of intent - describing reasons for seeking residency (electronic copy via email)
- Licensed or eligible for licensure as a pharmacist in the state of California
- If selected, the dates for on-site interviews are February 5 and 12, 2010
